Hold Harmless and Liability Release Form

Section 1

The undersigned, parents or guardians of the participant listed below, a participant of Future Engineers of America Pre-College Initiative Program, hereby release and agree to hold harmless The National Society of Black Engineers Birmingham Professionals, its officers and associated personnel including those of its affiliate organizations and sponsors, from any and all claims, liability, or damage to property to each of the undersigned,his/her parents, siblings, or heirs.

The indemnity and hold harmless agreement shall be considered a complete and total waiver of any and all liability on the part of Alabama Power or any other facility owner associated with the program, its employees, and property managers. The undersigned, parents or guardians of the participant listed below, acknowledge and completely understand that each participant will be engaging in a program of activities that demonstrate engineering principles. The undersigned release all liability of risk which may result from not only the participants actions, inactions or negligence, but also the action, inaction, or negligence of The National Society of Black Engineers Birmingham Professionals, its officers, associated personnel, affiliate organizations, and sponsors.

The undersigned, parents or guardians of participant listed below, understand that their child is not required to participate in the Future Engineers of America Pre-College Initiative Program. The parents and the undersigned participant do so willingly despite any risks and despite this Release.

Section 2

The undersigned, parents or guardians of the participant listed below, give consent to have a member of The National Society of Black Engineers Birmingham Professionals, associated personnel, members of affiliate organizations, and sponsor to call for medical assistance and/or treatment and agree to be financially responsible for the cost of such assistance and/or treatment. The undersigned, parents or guardians of the student listed below, further indemnify The National Society of Black Engineers Birmingham Professionals, associated personnel, members of affiliate organizations, and sponsors harmless for any such medical costs.
